REFEREE REQUIRED FORMS
There are several forms that must be provided to referee at the National Sports Center.
Required for All Sports
All referees, regardless of sport, must complete the following W9 tax form and return.
Soccer
Soccer referees must download and complete the following waiver/information form. Please download the form that is apporpriate for your location. You must also provide the following information when refereeing. All required forms for soccer events should be mailed to Bill Roll at 1700 105th Avenue NE, Blaine, MN 55449 or faxed to Bill Roll at 763-785-5699.
