The online waiver is to be completed by each player after team manager has filled out online roster.

2013 Walmart Hockey Cup 3v3 Liability Waiver

• Age verification required for all players
• Copies of birth certificates/passports or drivers’ licenses are required at Final Team Check-in
• Place in alphabetical order by players last name
• Teams may fax or scan and email prior to check-in or bring with them to Final Team Check-in

All coaches involved in a National Sports Center youth event are required to take concussion training as dictated by Minnesota Statue 121A.37. The U.S. Center for Disease Control and Prevention has a free, online course entitled “Concussion Training for Coaches (and referees)”, accessible by the link here.

• Teams are required to stay in the official tournament hotels.
• Provide the name, city and phone number of the hotel. Update as needed using the 2012 Walmart Hockey 3v3 Hockey Cup Team Management tool.
• Hotel contact information is used in an emergency. In the event that we are not able to contact you using your cell phone, we will contact you at the hotel.
• The National Sports Center is also required by the State of Minnesota to track for economic impact. Did you know that the National Sports Center is second only to the Mall of America for economic impact and the number of annual visitors?
