ADULT 4v4 LEAGUE REQUIRED FORMS

Team Roster

You can now manage your team's roster online making it easy to drop and add players and edit player information.

Liability Waiver

Each player should complete the online waiver prior to the start of league play. Please note that the team roster must be completed by the team manager before using the online waiver.

USA Hockey Registration

USA Hockey registration is MANDATORY for all Adult 4v4 League participants. Membership is $40 per player and must be paid when registering with USA Hockey. Visit USA Hockey BEFORE your first league game to complete your online registration.

 

Reminder: Print your registration confirmation bar code and bring to your first game.

 

USA Hockey Membership is an annual membership
