REQUIRED FORMS

Required forms are due by first game

Complete the online league roster (2009 roster not available).

  • »  Addresses required. Player addresses are directly linked to the NSC Liability Waiver as required by the State of Minnesota.
  • »  Final rosters are printed prior to Final Team Check-in as noted above and will NOT reflect addresses. 
  • »  A unique jersey number is required for each player; no jersey sharing permitted.
  • »  Provide each player’s primary position on final roster.

Complete the online NSC Liability Waiver (2009 roster not available).

  • »  Provide parents with the league ID number for this task. Players are not permitted to participate in the league without a completed NSC Waiver on file.
  • »  You may monitor the progress of your parents by selecting View Roster from the online roster menu. The WV column stands for Waiver. Each time a parent completes the online waiver, an ‘X’ is displayed in this column to the left of the player’s name.
  • »  Waivers are tracked to each player’s address, not the parent’s email address. Therefore, a parent may complete the online waiver from any computer!

Provide Proof of Age

  • »  Proof of each player’s grade presented at Final Team Check-in.
  • »  Spring report card or student ID accepted.

League Balance

  • »  The team leader or coach may pay the team balance online by Visa and MasterCard or by check or money order. One payment per team.
  • »  Indoor Lacrosse League balance due payment option
  • »  If you are paying by check or money order, mail your payment to NSC Indoor Lacrosse League, 1700 105th Avenue NE, Blaine, MN 55449. Be sure to record your team ID number on the check or money order.
  • »  If you are paying by cash, office hours are Monday-Friday, 9 am-5 pm. The main office is located in the Administration Building. Enter the building from parking lot B off Davenport Street and proceed to level 2/Plaza Level.
