Street Futbol 3v3 Application & Required Forms

Street Futbol challenges players to use their strengths and turn them into strategies on a smaller field. Do you have what it takes?

Team Manager Account

Complete the tournament roster using the Team Manager’s Account Link

All players must be identified on the tournament roster. Enter the player’s name, primary position and jersey number.

  • The player’s date of birth (DOB) is entered by the parent on the NSC Liability Waiver. Remember: You must enter the player names before parents can sign the waiver
    • Provide parents with the team ID and the link for this task.
  • You may monitor the progress of your parents by selecting View Roster from the online roster menu. Each time a parent completes the online waiver, an ‘X’ is displayed to the left of the player’s name in the WV

Liability Waiver

We are excited to offer you a new waiver experience, NSC Event HQ!  Please direct parents to complete their waiver using the process outlined below.  The account owner must be 18 years of age or older.  (Only one account per family)

  1. Create your account HERE, or log in to your existing account HERE and skip to the “Complete a Waiver” section below.
  2. After you have created your account, verify your email and set up your password.
  3. Log in to your account and add your children and or spouse as “participants”  (If applicable).


To Complete a Waiver

Log in to your NSC Event HQ account HERE

Once Logged In:

  1. Click on the “Customer Tools” link in the main menu bar and then select “Complete waiver”. 
  2. Select the event.  You may also filter the events by sport and/or name. 
  3. Enter the Team ID-provided by the team manager.
  4. Select the participant’s name from the roster (if not on the roster contact the team manager).
  5. Select the participant’s name from the drop-down in your NSC Event HQ account. If  participant is not listed in your account,  you must add them.  
  6. Read and accept.

PLEASE NOTE:  Managers/Coaches may NOT complete waivers for any player other than their own child.  If you do so, all waivers will be re-set and your account will be locked.

Player Fees

Player fees are payable by Visa, MasterCard, check, money order or cash

  • $165 team fee due with tournament application
    • Option to include exclusive camp for your team with the freestylers from Joga bonito for $30/team
  • Refund Policy
    • Teams dropping from the tournament prior to July 27th will receive a voucher in the amount of the team fee. The voucher is valid within one year of the event and can be used for other 3v3 tournaments. Using the voucher is based on availability and does not guarantee acceptance into the event.

Concussion Training

All coaches involved in a National Sports Center YOUTH event are required to take concussion training as dictated by Minnesota Statute 121A.37. The U.S. Center for Disease Control and Prevention has a free, online course entitled “HEADS UP” is accessible by the link here.

Proof of age

  • Proof of age is required for all players
  • Youth players are required to present proof of age at Final Team Check-in: Player pass, copy of birth certificate or passport.
  • Adult players must be able to produce proof of age, if challenged: Driver’s license, player pass, copy of birth certificate or passport.
  • Player pass from previous season may be used for youth or adult players.

Roster Guidelines

  • Rosters may contain between 3-6 players
  • Age groups:

Boys and Girls Divisions: (please note:  all 9U-14U teams must have at least one adult present during games in a supervisory capacity






All players must meet the age requirements and be listed on the team roster.  Please see the age categories for the tournament.

Final Team Check-in

Saturday, August 8, 2020

  • Location: Welcome Center
  • Time:  Opens at 8:30 am.  No later than 1 hour before your first game


  • A representative must check in 1 hour prior to your first game
  • Proof of age must be presented for all players; no proof of age – no play.
  • Boys and Girls Divisions: (please note:  All 9U-14U teams must have at least one adult present during games in a supervisory capacity
