Indoor Soccer League Application & Required Forms

Session I: November 8 - December 14, 2019
Session II: January 3 - February 22, 2020
Session III: February 28 - April 25, 2020

Required Forms

  • All players must be identified on the current team roster and have completed the online NSC Liability Waiver within 48 hours of game start time in order to be eligible for league play.
  • Rosters are frozen on the following dates. No player additions are approved after these dates:
    • Session I: November 22, 2019
    • Session II: January 17, 2020
    • Session III: March 13, 2020
  • Ineligible players will result in a forfeit.
  • Current official team rosters are available to the referees at the monitor’s desk in the event of a challenge. See rules.

  • You can manage your team's roster online, making it easy to drop and add players using your team manager account.
  • You will need your team ID number you received after registering.
  • Please note that the team roster must be completed before players can complete the online NSC Liability waiver. 

Liability Waiver

We are excited to offer you a new waiver experience, NSC Event HQ!  As a new user you will need to set up a household account in an adult's name. Click HERE to set up an account. 

  • Once you create your account and verify your email address, you can add "participants" to your account (children, spouses, etc.) for which you can then complete waivers. 
  • You will need to provide your name, birth date, gender, address, email and phone number
  • Once you add participants, future events are smooth and easy. You simply click and add, and you’re done!
  • You only need to set up a household account once – quickly sign in with your new username and password for future events
  • We will ask you to confirm your email to keep your account secure
  • You may now find your event/participant and complete the waiver

League Balance

  • The team fee balance must be paid 48 hours before the first game.
  • If you are paying by cash, office hours are Monday-Friday, 8:30 a.m.- 5 p.m. The main office is located in the Welcome Center. 
  • Payments will not be accepted at time of check in before the first game.

Concussion Training

All coaches involved in a National Sports Center YOUTH event are required to take concussion training as dictated by Minnesota Statute 121A.37. The U.S. Center for Disease Control and Prevention has a free, online course entitled “HEADS UP” is accessible by the link here.

Proof of Age

  • Proof of age is required for all youth players prior to first game.
  • Proof of age includes any form of Minnesota State ID, driver’s license, passport, school ID with birth date, birth certificate, and/or player pass.
