3v3 Freestyle Futsal: November 13 (Boys) - November 14 (Girls)

Team Manager Account

Complete the tournament roster using the Team Manager’s Account Link

All players must be identified on the tournament roster. Enter the player’s name, primary position, and jersey number.

  • The player’s date of birth (DOB) is entered by the parent on the NSC Liability Waiver. Remember: You must enter the player names before parents can sign the waiver
    • Provide parents with the team ID and the link for this task.
  • You may monitor the waiver progress by selecting View Roster from the online roster menu. Each time a parent completes the online waiver, an ‘X’ is displayed to the left of the player’s name in the WV

This is a youth event all waivers must be completed by the parent or legal guardian for each player.

Once the roster has been entered direct parents to complete their waiver using the process outlined below in the NSC Event HQ.  The account owner must be 18 years of age or older.  (Only need one account per family).

Proof of Age

Proof of age (POA) is required for all youth teams/players can use a Player pass, copy of a birth certificate, or passport. Email to Registrar

Adult players must be able to produce proof of age if challenged: Driver’s license, player pass, copy of a birth certificate, or passport.

Player Fees

Player fees are payable by Visa, MasterCard, check, money order or cash

  • $150 team fee due with tournament application
  • Refund Policy
    • Teams dropping from the tournament prior to November 6th will receive a voucher in the amount of the team fee. The voucher is valid within one year of the event and can be used for other 3v3 tournaments. Using the voucher is based on availability and does not guarantee acceptance into the event.

Concussion Training

All coaches involved in a National Sports Center YOUTH event are required to take concussion training as dictated by Minnesota Statute 121A.37. The U.S. Center for Disease Control and Prevention has a free, online course entitled “HEADS UP” is accessible by the link here.

  • Rosters may contain between 3-6 players
  • Age groups:

Boys and Girls Divisions: (please note:  all 9U-14U teams must have at least one adult present during games in a supervisory capacity

The NSC is happy to announce Remote check-in. This means teams will submit all required forms prior to the start of the event. Email to  Registrar  
No need for in-person check-in, no need to bring paperwork to the game, it will all be submitted prior.  Players who have not completed a waiver and/ or provided proof of age will be deleted from the roster and are ineligible to play.
Upon arrival prior to the first game, a team representative will view and freeze the event roster with the  onsite staff
All required forms are due by noon Friday, November 12
